No.1 Complaint about management of Snohomish county, WA, community health care clinics management and possibly all state health care management practice to WA state health department.
I (Catalin Pamfile) have a very high concerns about the management of community health care clinics on Snohomish country and possibly all community health care management system.
-There are high delayed on medical records process. My medical records has been process on 2 moths, never show up in the system in order to my primary doctor be able to see them.
-There are high delayed on referral process waiting for a CT exam for 2 moths, waiting for ultrasound exam for 2 moths, waiting for referral to see the urologist doctor specialist for 2 moths
-THERE IS A VERY HIGH CONCERN, AGAIN, VERY, VERY HIGH CONCERN about the practice management of the state and in this case about management of Snohomish county community health system about the treatment and policy and management overall system:
– the management practice include corrupting and changing the words on medical records like you can see in my attachment records on date: 05/05/2011 changing the word ?PAIN? with the word ?DISCONFORT?. When I ask the question who did that, I have received the answer that the personal receptionist does not have enough knowledge how to write that term ?PAIN?, I ask for them to be trained, than I have the answer that the management has the policy to tell them to do that and the history can continue ?.
-the management practice include give to all the people who has DSHS, GAU insurance a mental disorder test each time they go to the doctor visit with first answer that DSHS require that test, I ask for the written policy from DSHS, after investigation I had received the answer that: our clinic practice and general all the state clinics require that the test is given to all the people with DSHS, GAU insurance and so on??. If you look on this test on attachment you can see very easy that the questions included in this test can easy mislead the diagnosed of the patient and it can be used instead the patient with the purpose to be send to some kind of mental ill treatment or sleep apnea treatment like was happened with my friend who die because has been mislead his diagnose to sleep apnea for 3 moths and actually he has cancer and he die after 4 moths.
-the practice to call the patients at home by undercover mental ill doctor specialist and lie to the patients like my self saying that She ( Marci Bracich) is a ?care coordinator? and will help me out to get my treatment and She schedule an appointment with my self to do that. On the interview it was actually an interrogation practice with questions and statements like what is personal day by day life?, I have to many complaints about the clinic and so on ??.
-the practice to call the patients at home and tell them that if you do not like the system you have the option to do not accept the DSHS, GAU insurance and I ask her what other option I have to get my treatment because I can not work, she did not have any answer for me and also enforce that: this is the way the system works so I have to understand and do not have so many complaints about it.
I try to make a resume about the practice and do not go into many details but I can write a lot more details about each complaints I have. All this complaints has been discus with complaint manager of Snohomish county community health district Sheryl Moss and I received from her those answers mentions before on this letter.
New-on my medical records are missing my follow up records from others doctors, are missing my blood test on details like liver testing, thyroid test, and all other numbers and testing recommended for my health follow ups, on my medical records are missing also the picture from UW, GI doctor and the letter with my symptoms I personally give them to the doctor and she gave it to the nurse and I received the call from medical records that those papers are been scanned on the clinic computerized system and those records are not in the system.

No.2 complaint about Snohomish County Community Clinic Management, Falsification and missing of MEDICAL RECORDS to WA state health department
My name is CATALIN PAMFILE and I continue to complaint about Snohomish County Community Clinic Management, Sheryl Moss (425-789-3700).
I request my medical records two times from Lynnwood Community clinic and I realized on my MEDICAL RECORDS (please see attach documents) the same page in my medical records has different notes. When I request the first time my medical records on that page has on :

1. abdominal discomfort As usual, c/o epigastric/sternal pain???he believes he has an infection?.etc.

On ?Reason for visit? section is patient side words, when the patient make an appointment, on this case my self, I sad the reason for the doctor visit by phone witch it was:
– upper abdominal pain with concerns about my little middle chest bone witch is hurting me when I touch it.

Again this complaint has been discus with Sheryl Moss before and I send you (WA state QSQAC complaint intake) my first complaint. Now I discussed again with Sheryl moss and the new medical practice manager Shawn Frederick and the answer I have is that: this is the way we do medical care in USA, in Europe is different, here is different. I am really concern about the training Sheryl Moss provide for all these employees. My last visit with my primary (06/10/2011) Dr Dana Fang it was seriously affected about this kind the training because I notice words from Dr Dana Fang (you are very stress out, you do not know how the system is working here, let me explain to you how the things are working here), same words use by Sheryl Moss when I complaint about my medical records missing in the system, these words was never used before by Dr Dana Fang.
There are medical records missing on the system and the Dr Dana Fang do not has them and do not has the access to them, some of them 2,3 moths old. Medical records from testing done from referral from Dr Dana Fang like nuclear medicine test done on UW on my heart, several others tests from CDI, MRI, X-RAY, ultrasound, medical records from GI specialist Dr Inadomi from UW, digestive disease center, medical records to be follow up from my last primary doctor, Dr Lawrson in Edmonds, medical records from GI doctors in Texas and so on,??
Please help me, I need to fix my health and I can not work because my permanent upper abdominal pain, I am on well fare and I do not have other choice to find other primary doctor or other clinic with the insurance I have.

-missing and not been accessible to my primary doctor my medical records( the old medical records from texas and the new medical records from UW, Swidis, Sound family clinic, CDI).

-every doctor visit I have with my primary doctor on Lynnwood clinic I am force to complete the mental health test with the answer that DSHS require to do so (see attachment mental ill test).
-all my complaint to Sheryl Moss (425-798-3700) (the person who sad it is in charge of complaint department of Snohomish County Community health Clinics) are not taking care and also having more problems every time I am complaint to Sheryl moss about the management practice on Community Clinics I have the answer that: If I do not like it I do not have to accept disability lifeline insurance and this is the way the system is working in Community Health Clinics.

I attach previous complaint documents for records witch I has been talk with Bob Farrell about all these complaints and he sad that he will take care of these complaints.
The way Bob Farrell took care of my complaints are worse and in opposite side of taking care of my health and I can say and feel that mister Bob Farrell is a criminal director in the way he is taking care of patients. He is the director of Snohomish County Clinics and all the employee like doctors , under cover mental ill doctors, under cover social workers, nurses, managers has to listen to him and do what he sad.
The attitude of Lynnwood Clinic personal change in worse since I talk with Director Bob Farrell, (Dr Fang sad: you are in a lot of stress and if this MRI test does not show anything else I can not do anything for you), my medical records are still missing , my medical records continue to be falsify and change, I try to make appointments to show the medical records I have and the doctor appointment has been cancel by Shawn Frederick, my blood test are not complete and my testing are not continue.
I will summit step by step some of the information I have in separate e-mail so you can have an idea of what is going on in Community health System.
The root cause of these criminal practice I believe are orders from Director Bob Farrell to his own employees.
On this case are also involved: Director Bob Farrell, Dr Dana Fong, manager Shawn Frederick, Sheryl Moss ( who does not have any health education or specialty and make decision on my health, she is Bob Farrell employee who take care of complaints from patients like my self).

Reason(s) for visit 1. ongoing med issue Here b/c wants to review records. Does not seem to understand that we cannot give him copies of his records from other institutions. He feels that his records are incomplete.?
Well I can say that I feel like I am in hands of criminals director like Mr Bob Farrell and criminal doctor like Dr Dana Fang because I have the prove of medical records from CDI (see attach document CDI medical record) witch is the second attach document and you can see that in paragraph findings: ? FINDINGS: within the right lateral dome of the liver , there is a 5 mm calcification. There is very subtle decreased attenuation of the liver compared to the spleen, witch raises the possibility of fatty infiltration.? We can see fader down: ? Mild multilevel degenerative disc disease.?
So I have been looking for my missing medical records witch Dr Dana Fang does not care and do not told me anything about them. These are directions from MR Director Bob Farrell and medical practice from Dr Dana Fang witch I can see do not care about my health care.

On attach document medical records 06/10/2011, I can realize that: the orders are coming from Director Bob Farrell to the Dr Dana Fong because Dr Fong is writing some words I never sad to Dr Fong only I told to Mr Director Bob Farrell and Sheryl Moss on complaint about ?abdominal discomfort? instead of ?abdominal pain? on my medical records and the answer it was that the receptionist does not have knowledge and they have a set up system in computer to do that ( see my previous 1-st complaint I e-mailed to you and I attach document also with this e-mail).
On CHC medical record 06/10/2011 on first paragraph you can see: ? He is also upset that the phrase ?abdominal pain? does not show up on one of his notes and does not understand that ?abdominal discomfort? used instead, is a synonym.?
Lets look in dictionary: abdominal discomfort is an low abdominal pain you can handle it and do not affect your body movements, instead abdominal pain and permanent abdominal pain like I have is very hard to handle it and control and can affect your body movements.
When I request a doctor appointment I sad ?permanent abdominal pain or abdominal pain?, including the receptionist, the people on the street, the ordinary people understand the difference.
Well this not surprise me what kind the director Bob Farrell is and what kind the doctor Dana Fong is who does not know the difference between abdominal discomfort and abdominal pain because I see a lot more of poor and unacceptable quality of health care they provide.

For 6 moths I try to get my medical records from texas and WA and discuss with Dr Dana Fong my health situation and all the time she sad she does not have the medical records and I have to get them from texas. Well how about the medical records Dr Dana Fong order like my blood test, my nuclear medicine test on my heart on UW, how about my exams done on CDI and been order by Dr Dana Fong. All these complaints have been mention before on my previous complaints to WA state health department about my missing and falsify medical records.
I try to get some medical records on my own and talk with Dr Dana Fong about my medical records but like I sad in previous complaint my Dr appointment has been cancel by manager Shwan Frederick.
I attach on this e-mail some of my records from UW and CDI and I would like to know who will explain and talk to me about these medical records I have and the ones are missing also?

I, CATALIN PAMFILE, make a doctor appointment on ?09/07/2011?for a new referral to GI doctor because my last GI doctor, Dr Patterson, does not has knowledge of the liver, on Lynnwood community clinic and I continue to have continuously abdominal pain, burnings, chest pain and short of breath and I ask for the last doctor I see was Dr Balauag but I had appointment with Dr Avelino. On the last 2 moths I have been changed to see several doctors like: Dr Dana Fong, Dr Marlon Balauag, Dr Avelino and Dr Morgan Roystera and on all appointments I can see questions and clear focus about depression and health mental disorders on everyone of these doctors when I have the appointments with them and they are not focus to solve my health.
On my appointment with Dr Avelino I do not have any check on my longs, or on abdomen but after a long discussion, arguing and conversation about my medical records, and my missing medical records from the Lynnwood clinic witch abiously does not have any interest on focus on my health care, Dr Avelino agree on give me 02 refferals . One referral for hydroscan test for my gallbladder and other referral for GI doctor with knowledge of liver because the last GI doctor a had Dr Patterson does not have knowledge of liver.
After a long wait for my new GI doctor referral I called again the clinic about my referral and I received the answer that Dr Avelino want to see me again. I have been the second time 09/27/2011 and the conversation got worse.
How this Dr Avelino order a hydoscan when he is not a GI doctor, he sad is not a GI doctor and continuously delay my GI doctor referral.
I request the medical records and I see that on 09/07/2011 on the reason for doctor visit does not show, request for GI doctor referral. And also on assessment plan he is waiting for results from mGI and recommendations for continued surveillance.
Again falsify medical records, lie, mislead and so on, THIS IS THE KIND THE HEALTH CARE PRACTICE LYNNWOOD CLINIC PROVIDE.

I, CATALIN PAMFILE, make a doctor appointment, ?09/12/2011?, on Lynnwood community clinic for my continuously abdominal pain, burnings, chest pain and short of breath and I ask for the last doctor I see was Dr Avelino but I had appointment with Dr Morgan. On the last 2 moths I have been changed to see several doctors like: Dr Dana Fong, Dr Marlon Balauag, Dr Avelino and Dr Morgan Roystera and on all appointments I can see questions and clear focus about depression and health mental disorders on everyone of these doctors when I have the appointments with them and they are not focus to solve my health.
On my appointment with Dr Morgan Roystera I do not have any check on my longs, or on abdominal but a direct conclusion that We see patients with depression witch has your symphtoms and I should find a provider who can do your expectation.
I ask Dr Morgan if she knows about parasites like ?echinococcus? who can cause these damage in my liver ?5mm tumor?and ?residual thymic tissue? in my longs, low fever, etc,? and She sad that: ?do not worry about a small tumor in your liver, I had tumor on my liver and my foot also, I am Dr pathologist and of course I know about echinococcus?. Than I ask Dr Morgan for a blood test that can be done to find out if I have these parasite and She give me the bloodculture test and stoolculture test. When I been to do my blood test, the laboratory ask me if I know witch type of bloodculture, type 01 or type 02? And She call Dr Morgan to find out and the answer it was to do type 01. I ask the lab technician what is the difference between the two tests and I have the answer that type 02 is more complete test and the most of the doctors order type 02 but your doctor want type 01 because anyway they did not find anything wrong with you. Than I ask the lab technician if she knows about echinococcus and She answer that: She does not know but I can talk with the microbiologist about that. I ask the microbiologist about echinococcus and He sad that for echinococcus the doctor has to order a cirology blood test, the bloodculture and stoolculture does not cache echinococcus.
Now, what kind the Dr pathologist is Dr Morgan who does not know about the right blood test for echinococcus? Dr Morgan also order only type 01 bloodculture. Dr Morgan is focus on depression and mental ill symptoms and disorders. THIS IS THE KIND THE HEALTH CARE PRACTICE LYNNWOOD CLINIC PROVIDE.

09/29/2011 I had a conversation with Bob Farrell when I call on complaint phone line and request to make a complaint 425-789-3700 . The call has been transfer directly to Bob Farrell and I told him that nothing has change on his Lynnwood clinic since Dr Fong has been fire, I found the same practice: falsify my medical records, delay on my referral to GI doctor and I see all different doctors on each appointment I had on the clinic. He mention on this conversation that: ?DO NOT THREAT MY STAFF?. So I can see very clear that Bob Farrell is in charge of medical staff and give orders to medical staff without having a education or license on medical field. Now is also very clean to see Bob Farrell new strategies on given the orders on his staff to complaint about my self of threatening. I do not threat any of staff of Lynnwood clinic. I also can see that my relation patient-medical provider has to STOP IMMEDIATELY.
On previous complaint No.13 about Dr Avelino and No. 14 about Dr Morgan I have summit and explain the continuously practice of falsify medical records and delay on my referral and also more details about quality of medical service and knowledge I received from this community Lynnwood clinic.

There is a coordinated health system to do not provide the right health services.
I complaint to Lynnwood manager clinic, nothing change.
I complaint to community health plan, nothing change.
I complaint to provider one, DSHS, nothing change.

I request change of primary clinic provider (Lynnwood community health clinic) to community health plan, they delay the process for moths, no denied letter but not problem solved. I received a list with some clinics, most of them very far away and do not accept this insurance I have GAU.

My name is CATALIN PAMFILE and I want to complaint about the practice of Dr Cara Debley and add more information about director Bob Farrel case.
If You look on my attach documents You can see 02 letters from Dr Cara Debley, one letter to my self, Catalin Pamfile and second letter to my primary care Dr Dana Fong from Dr Cara Debley witch do not mach each other and have completely different information on them. Dr Cara Debley is telling me that She can not see me any more and at the same time is sending a letter to my primary Doctor Dana Fong that she will review the gastroentereologist records and also She mention about some type of delusional disorder that I need psychiatric evaluation. I request several times my medical records but I never received this page and also I see that this is page no 3 and I will like to have page 2 and 1 to see what else is behind. I do not know who need psychiatric evaluation if you send 02 letters with different type of information but if Dr Cara Debley does not need a psychiatric evaluation these letters confirm a very serious type of behavior. I attach some medical records with a need of gallbladder surgery, gallbladder distended, liver tumor, barrets esophagus, chronic gastritis just to have an idea how Dr Cara Debley care about my health. Now also because this medical record I did not receive it when I request my medical records on 05/2011, 08/2011 and I received it on 09/2011 and it is missing pg 01 and pg 02 that is a very concern about the health care I received from both side Community Lynnwood health clinic and Dr Cara Debley and it is a clear hiding, falsify medical records, miss lead health care and so on. This medical records looks a very clear set up with lawyers and others corrupted justice system advisers who is looking to cover up and destroy not looking for truth and fix the problems and it is also a very clear High level orders and directions. DO NOT BLAME ANYBODY WHEN GOD JUSTICE IS COMING CORRUPTED CRIMINALS

Denied access to my personal records
I request several times during 7 moths period access to my missing medical records and I had been denied the access. Medical records Community Lynnwood clinic 425-835-5200 I ask why I am denied the access to my personal medical records and the answer it was that :I can not give you the medical records from texas state, but I am asking for the copies of my own personal records. I called again the medical records on Lynnwood clinic and I sad that: I have the right to see my medical records and the answer it was that She has to talk with her supervisor in order to give me access to my medical records.
Now I am just looking to have copies from my own missing medical records on Lynnwood clinic since I have been receiving medical service to this Community Lynnwood clinic all the time the doctors like Dr Dana Fong sad that She does not have my medical records and I had asking at front desk for my medical records and medical records department and this problem is been going on for 7 moths and never been fix.i had answer like that is all they send us but did not show me any of missing medical records, I sign al over again released of medical records and the problem was not solved.
Now if you look on complaints I already send more than 16 of complaints you can see hiding information, miss lead health care, falsify medical records, delay on referrals and never some one care or fix it. The way is turning out is a back fire to hide more information, to add more unknown information and so on.

I, CATALIN PAMFILE, make a doctor appointment, ?09/12/2011?, on Lynnwood community clinic for my continuously abdominal pain, burnings, chest pain and short of breath and I ask for the last doctor I see was Dr Avelino but I had appointment with Dr Morgan. On the last 2 moths I have been changed to see several doctors like: Dr Dana Fong, Dr Marlon Balauag, Dr Avelino and Dr Morgan Roystera and on all appointments I can see questions and clear focus about depression and health mental disorders on everyone of these doctors when I have the appointments with them and they are not focus to solve my health.
So I make no 14 complaint to WA state department and I explain the behavior of Dr Morgan Roystera and the results of that no 14 complaint Dr Morgan Roystera IS NOT A LICENSE DOCTOR, dr morgan Roystera IS NOT A LICENSE NURSE PRACTICIONER, so how this Dr Morgan Roystera has a dr appointment with my self, sign a blood test for my self, filled up medical records on my medical records?
This is one more prove of the STUFF medical employees mr bob farresll has working on his clinics because also He, Mr Bob Farrell director of the clinics is not a license Dr or has any of health care education.

If You look on my attach documents You can see 02 letters from Dr Cara Debley, one letter to my self, Catalin Pamfile and second letter to my primary care Dr Dana Fong from Dr Cara Debley witch do not mach each other and have completely different information on them. Dr Cara Debley is telling me that She can not see me any more and at the same time is sending a letter to my primary Doctor Dana Fong that she will review the gastroentereologist records and also She mention about some type of delusional disorder that I need psychiatric evaluation. I request several times my medical records but I never received this page and also I see that this is page no 3 and I will like to have page 2 and 1 to see what else is behind. I do not know who need psychiatric evaluation if you send 02 letters with different type of information but if Dr Cara Debley does not need a psychiatric evaluation these letters confirm a very serious type of behavior. 2-nd complaint, MISS DIAGNOSE, to WA state health department about Dr Procaccini in Swidish Cherry Hill medical center Gastrointeriologie
My name is CATALIN PAMFILE and I want to add more information to the complaint I already send to you about Dr Procaccini in Swidish Cherry Hill medical center Gastrointeriologie.
I request my medical records from Swidish Hospital and now I can prove the MISS DIAGNOSE practice by Dr Procaccini on my endoscopy.
I have been diagnose 1 year ago in Texas in Harris County Hospital(see attachment document) that I have Esophagus barrets (esophagus metaplasia, cancer) and is was perform an endoscopy by Dr Procaccini (see attachment document) and the diagnose from this endoscopy perform by Dr Procaccini it says that I do not have nothing in my esophagus, ? no barrats esophagus found? (see attach document).
So I have to go to UW and see Dr Inadomi and perform second endoscopy after 2 weeks and Dr Inadomi confirm the Barrets Esophagus with biopsie in my esophagus.
I have follow up appointments with dr Procaccini and the visits it was not very respectful and professionalism when I told him about the diagnose I have from UW endoscopy also Dr Procaccinei, like I mention in my first complaint e-mail to WA state health Dpt., sad that? I am in to much stress and He can not????.
I am very concern about this kind the practice and miss professionalism and knowledge from Dr. Procaccini. There is human life in this octorr hands and a miss diagnose, miss respect
